Precision Pain Care is not a pharmacy and we do not accept patients solely to write prescriptions. Please see our Medication Management page for our position on pain medication.

What do I need to bring to my first appointment?

Any written medical history you wish to share with the doctors

A complete list of all of the medications and dosages you are currently taking. If you are taking any herbal supplements or vitamins, please bring a list of these as well.

You will have some paperwork to fill out that we will send you in advance of your visit, please fill this out and bring it with you. Download the paperwork here.

Your insurance card and driver's license.
